For seniors needing cardiac rehabilitation after surgery or illness, The New Jewish Home and NYU Langone Health have collaborated to create a state-of-the-art short-stay cardiac rehabilitation unit on the Manhattan Health and Rehabilitation campus.

Our specialty Cardiac Rehabilitation unit, located on our Manhattan campus on the Upper West Side, helps cardiac patients recover and regain strength so they can return home. This interdisciplinary program provides specialized in-patient rehabilitation for adults with heart failure and cardiac disease.

Drawing on years of experience with cardiac rehabilitation and using the latest clinical techniques and equipment, the Cardiac Rehabilitation Center helps seniors recover from cardiac issues and learn to best manage their health to stay healthy and active.

The Cardiac Rehabilitation Program

Cardiac rehabilitation offers individualized rehabilitation, exercise therapy, and support for seniors recovering from a cardiac episode or surgery. In addition to therapies, the cardiac rehabilitation program provides patients and their families counseling and education to help understand and manage the cardiac condition and maintain general health and quality of life.

Patient Education

A key component of the program’s success is patient education. The Heart Failure Program empowers patients, along with their families and caregivers, to be active participants in managing their health by:

• Recognizing when a doctor or emergency care provider is needed

• Providing education regarding medication management

• Managing stress and mental health issues

• Designing and maintaining a heart-healthy diet and weight management

• Understanding routine tests and treatments

Cardiac Rehabilitation Center is the first facility to be Joint Commission certified in Congestive Heart Failure.

The first Joint Commission accredited postacute heart failure program in New York State. Accredited in 2017 and reaccredited in 2019, 2022 and 2024.

The Cardiac Rehabilitation Team

A dedicated, caring team of specialist clinicians works with seniors and their families to create an individualized recovery plan. The entire care team is guided by this plan and by their professional expertise. The care team includes NYU Langone physicians (geriatricians, cardiologists, physiatrists), cardiac rehabilitation Nurse specialists, Physical and Occupational Therapists, Social Workers, Dieticians, Pharmacists, and specially trained Certified Nursing Assistants.

Cardiac Health

The cardiac rehabilitation program uses sophisticated telemetry cardiac monitoring equipment to give physicians and nurses warning of critical cardiac events and help the care team track a patient’s cardiac health.

Cardiac Rehabilitation Therapies

Cardiac rehabilitation therapies are offered seven days per week, based on the individual senior’s needs and health condition. In the cardiac rehabilitation program, seniors who may need additional health care services, a broad range of specialty physicians, and other providers are available onsite. Continuous intravenous medication services are available, as are pain management and wound care therapies.

Recovering and Going Home

When a patient of the cardiac rehabilitation program has recovered enough to return to her or his home, a plan is developed to support the senior’s safety and continued recovery. Coordinating with the senior’s family and physician, the care team prepares recommendations for services, such as in-home rehabilitation, nursing, and Home Health Aides, or other supports the senior may need. The goal is not just to help seniors recover and go home. It’s to help them stay well at home.

Recovery is Hard Work

After a day of rehabilitation and therapy, the Manhattan Health and Rehabilitation campus offers seniors beautiful gardens and public spaces to visit with family and friends. An extensive range of social, cultural, and recreational activities are available. There is free Wi-Fi for seniors and their visitors. On staff, clergy provide spiritual support for seniors and families, and regular religious services are offered for multiple faiths.
